L-Methylfolate Dosage

Dosage Guidelines for Hyperhomocysteinemia

No clear L-methylfolate dosing guidelines exist for hyperhomocysteinemia. Folic acid doses between 0.5 mg and 5 mg have been used for hyperhomocysteinemia, although doses between 0.8 mg and 1 mg are probably sufficient for most people. Doses above 1 mg do not seem to provide any greater benefit, except in people who have genetic conditions that cause excessively high homocysteine levels.

L-methylfolate has been used in combination with vitamins B6 and B12 (Metanx®) for nutritional supplementation in people with hyperhomocysteinemia who are also at risk for, or have, leg or foot ulcers. The recommended dosage is either one Metanx tablet twice a day or two tablets once a day. Each tablet contains 3 mg of L-methylfolate. The manufacturer of Metanx states that each tablet is equivalent to 19 mg of folic acid.

Deplin® is a medical food that contains L-methylfolate. It is approved for use in people with schizophrenia or who have hyperhomocysteinemia. The typical Deplin dosage is one 7.5-mg tablet or one 15-mg tablet daily. The manufacturer states that one 7.5-mg tablet is equivalent in the body to 52.8 mg of folic acid.

Dose of L-Methylfolate for Folate Deficiency

Typical folic acid doses for folate deficiency are 250 mcg to 1000 mcg (0.25 mg to 1 mg) each day. In severe cases, doses may be as high as 1000 mcg to 3000 mcg (1 mg to 3 mg) per day. Similar L-methylfolate dosages would likely be sufficient. If you have a folate deficiency, talk to your healthcare provider about the best amount of L-methylfolate for you.

L-Methylfolate Dosage Guidelines for Diabetic Peripheral Neuropathy

L-methylfolate is used in combination with vitamins B6 and B12 as a nutritional supplement for people with diabetic peripheral neuropathy. Metanx is a medical food approved for this use. The recommended Metanx dosage is either one tablet twice a day or two tablets once a day. Each tablet contains 3 mg of L-methylfolate. The manufacturer of Metanx states that each tablet is equivalent to 19 mg of folic acid.
